As the participant or parent/guardian of the participant(s), I hereby assume all risks and hazards incidental to the conduct of the activities at Reach Out Reptiles. I understand that physical activity carries the risk of harm and injury. I understand that in handling and being in the presence of animals there does exist a risk of injury including physical harm caused by the animals or the transmission of disease therefrom. I present that I/the named minor(s) have received all vaccinations recommended by my/their physician. I understand that no insurance coverage for participants in these activities is provided by Reach Out Reptiles. In consideration for my/the named minor(s)’ participation in Reach Out Reptiles programs, I agree that on behalf of myself/the named minor(s), I assume the risk of the activities and I release, discharge, indemnify, covenant not to sue, and hold harmless Reach Out Reptiles, its agents, employees, officers, and directors from any and all claims, causes of action, or demands, if any, in connection with the same, based on damages or injuries which may be incurred or sustained by myself in any way connected with said participation in activities or presence at Reach Out Reptiles. I give the employees and/or agents of Reach Out Reptiles authority to seek emergency dental/medical and/or surgical transport and treatment for myself/the named minor(s) if I am unable to do so. I understand that I will be solely responsible for any loss or damage, including personal injury, property damage, or death, that I sustain while participating in the Experience and that by signing this agreement I HEREBY RELEASE Reach Out Reptiles of all liability for such loss, damage, or death. I further certify that I/the named minor(s) am/are in good health and have no conditions or impairments which would preclude my/the named minor(s) safe participation in the Reach Out Reptiles Experience.

As the participant or parent/guardian of the participant(s), I hereby grant permission to Reach Out Reptiles and/or other participating parties to take and use photographic images or video content of myself/the named minor(s) during this program for the express purpose of publicity regarding Reach Out Reptiles programs, or otherwise at the discretion of Reach Out Reptiles.

I hereby acknowledge this release from liability for accidental injury or illness which I may incur as a result of participating in any activity. I hereby assume all risks connected therewith and consent to participate in this Experience. I agree to disclose my/the named minor(s)' physical limitations, disabilities, ailments, or impairments which may affect my/the named minor(s)' ability to participate in this Experience.

I have carefully read this release of liability and assumption of risk agreement and fully understand and agree to its contents. I am aware that by signing this document, I am giving up certain legal rights, including, without limitation, the right to sue Reach Out Reptiles, its employees, officers, directors and agents, on behalf of myself, the named minor(s), my child(ren), heirs, family members, personal representatives, and executors. I nearby release, covenant not to sue, discharge, and hold harmless Reach Out Reptiles, the owners and agents, and representatives, of and from the claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating to the participation or attendance of Reach Out Reptiles Experiences whether such risks are known or unknown. I understand and agree that this release includes any claims based on the actions, omissions, or negligence or Reach Out Reptiles, its employees, agents, or representatives. This specifically includes any damages or injury caused by any of the animals owned or maintained by Reach Out Reptiles.
